Adult Day Health Center that provides a customized program for seniors and vulnerable adults. It provides the following services: assistance with ADL and IADL, daily nursing monitoring and medical assistance, enhanced levels of care for those with additional needs, care coordination and assistance with community referrals, meals, transportation and daily therapeutic activities and social events.
This adult day health program is staffed by licensed nurses, personal care aides, an activity director and program director. It provides nursing care, medication management and the coordination of health care needs. Staff assist with personal hygiene, ambulation, feeding, transfering and other activities.
