These partners share our vision and help us meet people where they are at, in their communities, and provide them with services to support their health and safety.

This year, our care managers met in the homes of thousands of individuals and families, helping to coordinate a wide range of essential services. We further expanded our transportation and housing support services, launched new culturally appropriate food access and meal initiatives, and hosted a global LGBTQ+ Elders Conference, all to address health-related social needs.
