Adult Day Health Center that provides a customized program for seniors and vulnerable adults. It provides the following services: assistance with ADL and IADL, daily nursing monitoring and medical assistance, enhanced levels of care for those with additional needs, care coordination and assistance with community referrals, meals, transportation and daily therapeutic activities and social events.
