If you or a loved one have been diagnosed with dementia, you may qualify for support through the federal government’s new GUIDE (Guiding an Improved Dementia Experience) program.

What is GUIDE?
GUIDE is a program developed for Medicare dementia clients and their caregivers to offer increased support and education to individuals with mild, moderate, or severe dementia. The goal of the program is to provide compassionate, person-centered support for the dementia client and their caregiver that promotes quality of life and reduces stress.

Our behavioral health care management team is highly skilled and specially trained to manage the many challenges individuals face when living with mental illness. Specialized staff work with older adults who have mental health concerns and need in-home care services to develop individualized plans of care to help them maintain their independence and continue to live at home.

We’re thrilled to introduce BetterAge, a free online tool developed through a collaboration between AgeSpan’s Healthy Living Center of Excellence and BetterAge. It’s designed to assess your loved one’s well-being across multiple dimensions, offer personalized recommendations, and help you uncover ways to enhance their quality of life.

What’s even better? It’s built on expertise from fields like aging services, positive psychology, gerontology, and healthcare, so you’re getting advice backed by science and real-world experience.

Healthy eating, staying active, keeping in contact with friends and loved ones, and exercising the mind are essential for health as we age. AgeSpan’s Healthy Living Center of Excellence provides more than 16 evidence-based programs to help adults of all ages manage chronic health conditions like diabetes and arthritis, prevent falls, stay physically active, deal with depression, and take care of themselves as they care for loved ones. Programs are delivered statewide through a network of more than 80 community-based organizations, through in-person groups, online workshops, and 1:1 telephone calls.

AgeSpan’s Healthy IDEAS is an evidence-based program that helps older adults recognize and manage symptoms of depression through one-on-one support offered in person, by phone, or virtually.
