It assesses older adults and collaborates with their clients and families to develop appropriate personal care plans. They have the Aging Life Care Expert Search, an online directory of professionals who are providing care management services for elders and their families in the New England area. This chapter covers MA, NH, ME, VT, CT, and RI.
