United Healthcare SCO is a health plan available in Massachusetts (Senior Care Options) that combines Medicare and Medicaid services with social supports. It covers many services including, but not limited to, primary care and specialty physician visits, regular preventive health care services, prescription drugs, inpatient hospitalization, nursing facility care, community long-term care, emergency care, and transportation for medical care and other services indicated in the individual’s care plan. It offers individualized care that includes a personal care plan, specialized geriatric support services, and 24-hour access to a team of professionals who can answer questions. It also provides rehabilitative therapies, medical equipment, home care services, and respite care for elder’s family or caregiver.
