This is an intensive short-term rehabilitation program provided by an interdisciplinary team of nurses, physicians, physical and occupational therapists who work in conjunction with dietitians, social workers and speech and language pathologists. The goal is for clients to experience a smooth transition from therapy programs to their home environments. Living arrangements are available in private and semi-private rooms.

This program is located on the campus of D’Youville Life and Wellness Community.
