About AgeSpan

AgeSpan is a private, nonprofit organization that has been serving communities since 1974. We help older adults, people of all abilities, and caregivers find practical support to live safely and independently at home and in the community, including services like home-delivered meals, home care, and care coordination. It is a Massachusetts Aging Services Access Point (ASAP), an Area Agency on Aging (AAA), and designated Protective Services Provider.

To ensure everyone’s choice to lead fulfilling lives as they age.

A future where everyone can choose to live and age well, in communities that support them

Respect, Choice and Self-determination, Independence, Diversity, Equity, and Inclusion, Innovation, Impartiality, Compassion, Community, Collaboration

We take a whole-person, non-judgmental approach to assessing individual needs. Then we connect consumers and caregivers with information and services to address their needs, with ongoing support.

Helping people remain active members of the community as they age is at the core of our mission. We maintain close connections to community organizations and healthcare providers, working collaboratively to ensure consumers have the right services at the right time.

Family discussion

AgeSpan is known for developing and delivering innovative programs that address unmet needs in creative, new ways. Our approach is evidence-based and focused on meeting the real-world needs of people at every stage of their lives.

We vigorously advocate for seniors, people with disabilities, younger people in need of assistance, and their caregivers. When there’s an urgent or critical need, we go above and beyond to make sure it is met.

Our work to promote health across the lifespan isn’t done in isolation. AgeSpan takes a cross-sector partnership approach, leading networks of organizations to create a full-service/whole person entry point for health-related social needs.

AgeSpan works in close collaboration with a variety of community networks, including:

  • Area Agencies on Aging
  • Aging Services Access Points
  • Councils on Aging
  • Home care providers
  • Housing providers
  • Food banks
  • Transportation vendors
  • Benefit enrollment specialists
  • Behavioral health providers
  • Addiction treatment professionals
  • And others

As a network integrator, we have the capacity to work across payers and providers to promote health equity, reduce emergency department visits, and avoid unnecessary healthcare utilization.

The 28 members of this coordinated care network deliver home and community-based services to tens of thousands of individuals every day.

The Healthy Living Center of Excellence provider network provides evidence-based programs to help adults of all ages manage chronic health conditions and lead healthier lives.
