This program offers nursing care as needed with RNs and certified nursing assistants on staff including weekly glucose checks as ordered, diet instruction, and monthly in-services by nursing staff. The program provides assistance with activities of daily living, including showers as needed; 2 meals each day; 2 snacks; individual and group activities geared to client’s abilities; trips to local restaurants and parks; musical guests and performers; and women’s and men’s support groups. Services include communication with primary care physician; medical appointment follow-up including required annual physical; and family conferences as needed. Physical, occupational and speech therapy can be arranged as prescribed by participant’s physician or therapist.
