Best Home Health Care, Inc is a Group Adult Foster Care (GAFC) provider offering daily personal care and homemaking services to MassHealth members who meet the clinical criteria for the program and live in an approved environment. All other services: daily personal care, homemaking, case management, medication management only available to those involved with GAFC.

Eligibility
Older adults age 60+ and adults with disabilities who live in a GAFC approved housing. Residents must be eligible for MassHealth and need assistance with at least ONE daily personal task.
