The Community Family
This program offers both social services and skilled nursing care including med management/administration, diabetic care, medical monitoring, personal care, caregiver supports, and coordinated care with health providers. Small breakfast, hot lunch, and afternoon snack provided daily. The Caregiver Support Group is held on the third Wednesday of each month from 6-7pm and requires prior registration to attend.
