MassHealth – Transportation Authorization
A MassHealth member must meet the above eligibility requirements to qualify for transportation. The medical provider must authorize the need for transportation by completing a PT-1 form (Prescription for Transportation) online. A PT-1 form is required for each doctor that a member visits. Once transportation has been approved, the following numbers should be called to arrange a ride: for Greater Lawrence and Greater Haverhill, call Cape Ann Transportation Authority at 1-800-830-5191; for Greater Lowell, call Montachusett Regional Transportation (MART) at 1-800-854-9928.

Eligibility
Must be a MassHealth member traveling to/from a MassHealth provider for a MassHealth reimbursable service; must be unable to use public transportation or be without access to public transportation; must be without a family member or friend who could drive
